I get what the writers are going for with "Perry." It's supposed to add a level of intimacy, but at the cost of believable characterization, which is super important to many fanfic readers, myself included. Sticking with "Dr. Cox" seems like the best option, especially with a character like JD. I've only seen maybe two seasons of Scrubs, but I'm trying to picture this and it's making me cringe.
